CMD Standard of Care Online Family and Affected Individual Survey

We need you! Cure CMD and TREAT-NMD have teamed up to launch a CMD Standard of Care (SOC) initiative led by Drs. Ching Wang, Thomas Sejersen and Anne Rutkowski.  The CMD SOC workshop will take place Nov 14th-16th with the goal of publishing a CMD SOC document in 2010.  We need your perspective on the CMD care issues you currently face or have faced. Results of the survey will be presented to the CMD SOC committee.
